                      6.3.5    The competency of senior staff can be evaluated by :           LD/ HOU

                               a)  Working experience at least one year in  the
                                  relevant fields or

                               b)  Involved in the development, validation and
                                  verification of laboratory testing or

                               c)  Satisfactory and in the acceptable performance in
                                  previous proficiency program.


                      6.3.6    Check the scoring and give assessment according to           LD/ HOU/ OIC
                               the established criteria.

                               Competency criteria :

                                   •  > 85 %    -   Competent & able to train
                                   •  75 -85%  -   Competent
                                   •  50-74%   -   Competent with supervision
                                   •  < 50%     -   Need training

                      6.4.0    TRAINING PLAN
                      6.4.1    Prepare training plan at respective unit and submit to           HOU
                               HOU  for approval.  Staff  training  need  were  identified
                               by :

                                  a.  Programme evaluation
                                  b.  Directive  from Ministry of  Health  or Makmal
                                      Kesihatan Awam Kebangsaan
                                  c.  Staff  training  need  from  Laporan  Nilaian
                                      Prestasi Tahunan (LNPT)
                                  d.  Changes in staff job scope
                                  e.  Changes  in test methodology,  equipment,
                                      specialty and procedures

                      6.4.2    Review and submit to Laboratory Director for approval.         HOU/ OIC

                      6.4.3    Review training plan at least once a year.                   LD/ HOU/ OIC

                      6.4.4    Conduct training as per schedule.                              LD/ HOU/
